DOB varchar(255),

UNIQUE INDEX ( ID )
);
CREATE INDEX indexName ON tableName (ID, LName, FName, LOC);
CREATE UNIQUE INDEX indexName ON tableName (ID, LName, FName, LOC);
DROP INDEX indexName ON tableName;
